This practice does not provide emergency or crisis services.

If you are experiencing a medical or psychiatric emergency, call 911 or go to your nearest emergency department. For mental health crisis support, contact 988 (Suicide & Crisis Lifeline) or local emergency services.

4. Electronic Communications

This website may allow submission of inquiries through a contact form.

Please note:

  • Submission of a form does not establish a provider–patient relationship.

  • Do not include sensitive medical or personal information in website forms.

  • Email communications may not be fully secure despite safeguards.

Secure patient communication occurs only through the designated patient portal (TherapyNotes) after services are established.

Fax communications are managed through secure systems (Spruce Health).

5. Screening and Eligibility

All new inquiries undergo a screening process to determine appropriateness of services.

Submission of an inquiry does not guarantee evaluation, scheduling, or acceptance as a patient.

The practice reserves the right to decline services if a request falls outside the scope of practice.

6. Insurance and Financial Information

Insurance participation and financial policies described on this website are provided for general informational purposes and may change.

Verification of benefits and financial responsibility occurs individually prior to scheduling.

For self-pay services, Good Faith Estimates are provided as required by federal law.
